My nephew got hit in the forehead with a knob of the door, few days later it's still very swollen and still black and blue, when should you worry ?

Don't worry: The same thing happened to my nephew and my own son. It's very common and as long as your nephew is acting normal and not having any problems with nausea, vomit, off balance, disoriented, blurred vision or double vision and things like that, he'll be fine. The bruise can take quite a while to go away. If it doesn't go away in a couple of weeks then he should see his pediatrician.

Trauma: If there hasn't been any loss of conciousness or headache you needn't worry. The black and blue mark will change color over time as the blood resolves

Don't !: If he's acting OK and not having any vomiting or change in disposition then it's a big bruise. These often take weeks to go away completely

Forehead injury fore: The bruising and swelling are not the concern. The forehead has a lot of blood vessels (very vascular) and small injuries can make big marks. At the beginning after the injury, it is important to watch for dizziness, vomiting, confusion and other signs of concussion. I hope this helps.

Up to 1 month: Bruising usually resolves in 7-10 days. If there is a bruise or hematoma under the skin it will take longer. Similarly if there is trauma to the fat layer swelling may last longer. The important thing is to watch for any bleeding under the skull bone or signs of injury to his brain. If he has increasing pain, vomiting or behavior that is not normal have him checked.
