Not unacceptable.: Assuming that liver function is stable (INR is not increasing related to acute liver issue) then INR of 2.2 carries low risk of bleeding from IM injection. Would certainly be careful about where you are injecting and that injection is "required" rather than something that could be given orally or rectally. Generally surgeons can perform elective surgery when INR is less than 2.0
