Yes: Yes a doctor needs to look at a lesion to diagnose it .Lips can catch STD .Be careful where you kiss

Not typically: It usually starts with a painless ulceration, which is typically genital. Later, you can develop a diffuse rash. Syphilis can do all sorts of strange things, and if worried, getting tested is a good idea. Itchy lip lesions are not typical.
