Fasciculations: involving the tongue in addition to limb muscles would be more typical of ALS. The development of both upper motor neuron and lower motor neuron signs and symptoms is also an important feature of ALS. Upper motor neuron symptoms include increased muscle tone, spasticity, increased reflexes and pathological reflexes ( up going toe sign) Lower MN sign include atrophy, decreased tone and reflexes.
