Urologist: Given your concerns with chronic testicular pain, I would recommend you see a urologist for thorough examination and possible ultrasound of the scrotum.

Urology: may be epididymitis or low grade orchits resistant to doxy. Ask you physician about a course of Cipro (ciprofloxacin) or any quinolone Rx. I suggest that 1 year is too long and you will need tight urological follwup and testicular ultrasounds coupled with antibiotic Rx.
