Depends: If the incision was for circumcision it could be normal for the underside to have more bleeding and scabbing. What was the reason for the incision?

Swelling yes: As gravity causes the swelling to pool on the underside. By the same mechanism the proteins and platelets that form the "scab" can pool on the underside. But you haven't even mentioned why you had the surgery/incision. If you elevate the penis you can reduce the swelling.
