Primary HPT: Hi. Assuming your 24-hour urine calcium has confirmed primary hyperparathyroidism (1-HPT), the decision to do parathyroidectomy or just watch is debated. Personally, unless you have a high surgical risk we're not aware of or your demise in the short term is likely, I'd recommend surgery. If the offending parathyroid(s) can be localized with sestamibi/ultrasound, limited surgery is an option.

Hyperparathyroidism: Primary hyperparathyroidism caused by parathyroid glands in back or the thyroid. This form is usually treated surgically. A med that mimics calcium to inhibit Parathyroid Hormone secretion, Sensipar may help. Drink water to avoid kidney stones. http://www.mayoclinic.org/diseases-conditions/hyperparathyroidism/basics/treatment/con-20022086 http://www.ncbi.nlm.nih.gov/pubmedhealth/PMH0002195/
