Male breast: lesion. If it was ultra sounded two years ago and diagnosed as a lipoma, it may or may not still be a lipoma now. Regardless it is painful, that is a criteria for removal. I would see his doctor and get a referral to a surgeon to get it removed. This is a fairly straight foreword procedure. It will stop the pain, get the lesion out, and send it to pathology to make sure it is nothing else.
