But far and large the most common cause of chest pain in teens is musculoskeletal, meaning a sprain or strain in the chest wall muscles, often from a cough, is causing pain, often upon breathing in. This kind of pain is sharp, fleeting, and resolves on its own.

Not likely heart: Heart disease in a healthy teenager is of low likelihood unless there is strong family history and/or risk factors such as tobacco abuse, cocaine abuse, etc. Common causes of chest pain in an otherwise healthy teenager are anxiety, costochrondritis, or acid reflux. Very athletic teenagers (e.g. football or basketball players) can develop cardiomyopathy but it rarely presents as chest pain.
