Varicocele: The veins around the testis get distended sometimes with loss of their usual elasticity and drain poorly into the veins , become the same way you descried. Surgeons try to deal with it non-operatively by recommending the patients to wear underpants with high elasticity. But if it is causing pain, then it could be surgically treated by the surgeon. It is good to avoid prolonged standing.

Varicocele: Orchitis will give you sever pain in the testis just like when you have infection somewhere else such as on your face or ear lobe. This sounds more like varicocele. Varicocele is like varicocele is like varicose veins in the legs. it is a bunch of dilated veins around the scrotum. It is usually painless and when you lie down, or elevate the scrotum or you body the veins empty and it gets smaller
