Perforation of TM: She has a perforated tympanic membrane. The acute infection leading to the perforation probably started 4 days ago when she complained of earache. If she not bleeding a lot, you could wait to see her doctor in the morning. She will need to take antibiotics and probably be referred to an ENT doctor. In most cases, the tympanic membrane will heal well. Close follow up is needed.

ENT ASAP: Get her to a Pediatric Ear, Nose and Throat physician right away for a formal examination. Could be an infection, or could be she stuck something sharp in there.
