Yes & Yes: High lactate level at 4.1 is always pathological. It is a sign of tissue hypoxia (lack of oxygen) from various reasons and anaerobic metabolism due to lack of O2. Are you on Metformin for DM? Is your Creatinine >1.5.? In either case stop Metformin (side effect is lactic acidosis especially in kidney impairment). Is your blood pressure too Low? You need to talk to your PCP for complete analysis
