Specialist: Your best bet is to see your primary care physician, who knows you best. If they feel you need referral, then see the specialist.

Specialist: Generally the appropriate time for specialist care is when a primary care doctor is unable to provide the diagnosis or treatment/ follow up for a medical problem. Most primary care doctors have a very good sense of their limits, and are diligent to refer when appropriate.
