Usually painful: Thrombosed hemorrhoids are usually quite painful. The best thing to do would be to have it evaluated by a general surgeon or a colorectal surgeon. I am sure that there is a hospital near you that would have a person you could see. You could see him/her as an outpatient.

Likely Thrombosed: From your description it sounds like you may have a thrombosed hemorrhoid. These can be treated with analgesics, sitz bath, stool softeners and sometimes topical steroids. If pain is severe then surgery is quite effective at quick relief. Given your age you should see your doctor for definite diagnosis. You should have had a colonoscopy at age 50.
