Wean off Topamax (topiramate): You should wean off Topamax (topiramate) by 25 mg every 4-6 days to prevent side effects and ameliorate Topamax (topiramate) withdrawal symptoms. The post withdrawal period may take from 2 weeks to 2-3 month. Everybody seems to react differently. It's always best to make medication changes under a doctors instructions.
