Anxiety disorder: is state of tension, apprehension, worry, uneasiness or fear. It may be related to anticipated danger or arise for no apparent reason. While mild episodes of anxiety are common and do not usually require treatment, more severe forms can be chronic and debilitating. Hope it helps.

Refraction needed: You are in the early phase of developing a need for eyeglasses. At your age the need will not be great but when you push yourself you can get in focus, but when relaxed the basic power cuts in. It this starts to become disturbing, see your ophthalmologist for a proper pair of glasses (or contacts).

Need a refraction: You need an eye exam to rule out a refractwive error or muscle imbalance. It sound like you suffer from uncorrected hyperopia which a refraction would uncover. A phoria which indicates a muscle imbalance could also cause your symptoms. A routine eye exam would include evaluation of both conditions.
