Other causes: Sometimes an inguinal hernia that is not large enough to descend in the scrotum will give referred pain to the testis. Ultrasound of that testis will be normal. I am referring to a direct inguinal hernia & also to a small indirect hernia. Diabetic neuropathy can cause testicular pain. Rarely the pain is idiopathic (cannot be explained) and a scrotal support from an athletic goods store may help.
