Depends: It could mean a benign endometrial polyp, or some ovulation issues. If you are on the pill it could be breakthrough bleeding which is common and not worrisome. If it is every month it is more likely to be something structural (eg polyp or fibroid.) if it is just once or rarely, you probably don't need to do anything about it.
