Proper nutrition: BMI is only a guideline. It is important that you eat regularly, healthy, exercise and be happy.

Rash or irritability: Most food allergy presents with eczema - an allergic rash. Vomiting, diarrhea, poor weight gain, irritability or colic are other common presentations. Sometimes respiratory symptoms as asthma and ear infections may also be related to food allergy. Rarely blood in the stool may be caused by a food allergy.

Prick skin testing: The most accurate method of diagnosing a particular food allergy is a combination of the history and prick skin testing (typically preformed by an allergist). Blood testing is an alternative method though not as accurate.

Unlikely: I suppose it depends on what you are allergic to and whether or not you are able to eat a well balanced diet, rich in fruits and vegetables despite your food allergies.

How can a pre diabetic person with BMI < 18 gain healthy weight? I have lost 5 lbs since diagnosis.

BMI is a guide...: And not necessarily an absolute measure of health. If you're doctor is saying you must gain weight because of your low bmi, then you should focus on eating lean, healthy proteins like fish, chicken and turkey, and doing some resistance exercise (weights, etc). If this is a big concern, i'd strongly recommend working with a nutritionist (along w. Your pcp)- they can be a big help.
