Stop Self Treating: First you do not start antibiotics without evidence of infection. That is unwise, dangerous and unnecessary. Second, it appears you treated the wound appropriately. And three, you are up to date for the Tetanus. You do not need to go to the ER.

Puncture wound: No need to go to the ER as long as the wound is clean, you are on antibiotics and your tetanus shot is up to date.

No ER : But if the nail went through the sole of the shoe you are at risk of a pseudomonas infection from the glue in the shoe. Your regular doc can order that as it sounds like your first aid was great!
