Transitional means : That the vertebra is in between a full lumbar one or sacral one which is defined from counting down from the top of spine & it usually plays no role in terms of any symptoms unless there is a partial attachment to the sacrum or complete attachment leaving the first disc space at or above the top of the pelvis or iliac crests leading to altered mechanics of the spine known as Bertolotti's Syndrome
