See headache doc: If you do not have a headache specialist let me recommend you get one. Sounds as if you may require some kind of prophylaxis as opposed to just straight acute treatment which is really all an ER is set up to offer someone with a 3 day migraine. Also, why are you on metformin? Ovulatory stimulation? It can cause some bad headaches by itself.
