See a surgeon: A general surgeon does not necessarily mean you need surgery, but their training makes them experts in abd pains. A good History and physical may reveal possible causes, for example, if you ever had surgery, you may have adhesions in that area and when coughing, the "valsalva maneuver" is causing referred pain in the area, etc. Best wishes.
