Ovary size: Ovaries are usually not completely spherical, so size is best assessed by volume measurement. This is calculated using measurements in 3 dimensions, e.g. length, width, and height. An ovary with a maximum diameter of 4.5 cm is not necessarily enlarged, it depends on what the other 2 measurements are. I wouldn't diagnose ovarian enlargement with one measurement, it can be misleading.
