What kind of lab do all hospitals have, and why do they only test for 3 or 4 things in er triage and not more when a person is really sick?

Depends: All hospitals in developed nations like the US have labs capable of fairly extensive testing. In the ER, their goal is to rule out the life threatening diseases. They are really only looking to address those types of problems in that setting which is why their testing is very specific and focused. You can feel very sick, as many people do with the flu, but not be life-threateningly ill.
