At 23 not likely : but Squamous cell carcinomas of anal canal mimics anal fissure , could be missed if biopsy is not done in long standing cases and in older age group ( not at 23 ) . Anal fissure of 5 years is too long speak to your doctor for alternative therapy svn surgery and biopsy of the area .
