Tendinitis : If anti ccp is + think rheumatoid arthritis. But given history it could be reactive arthritis to infections and immune related due to hep c. Complete arthritis work up is needed for RA, REactive Arthritis . Is not symmetrical and lasts long time every morning I recommend Mod work up . But from description I would think of tendinitis just under thumb Palmer side. Local steroid injection gives +++
