Could heart bypass surgery require opening up of the chest?

Yes,it does: Heart bypss surgery does require that the surgeon open the chest,to be able to work on the arteries which feed the heart(and would be unaccesible otherwise)

Yes: Most CABG procedures still require opening the sternum (breastbone) to some degree. While thorascopic surgery is done, it is not mainstream, and usually requires an access thoracotomy (small incision) for completion.
