No urinary sensation: Loss of sensation of need to urinate suggests a neurologic deficit. This, combined with back pain could indicate low spine problems - from a pinched nerve to slipped disk to an abscess in the area putting compression on nerve roots. Any loss of neurologic function like this needs investigation. Doc needs to do good exam to see if any other neurologic deficits, and then may need MRI or CT of area.
