Possibilities...: Consider anal fissure, hemorrhoid(s), proctitis (inflamed rectum), anal abscess or fistula, juvenile retention polyp, trauma to the anal canal, skin changes around the anus (benign or malignant), condylomata. The only way to find out, and to start appropriate treatment, is to get examined. See your doctor please for confirmation of diagnosis first, especially since empiric therapy hasn't worked.
