Several mechanisms: Severe high blood pressure can cause the heart to perform less efficiently causing "cardiogenic pulmonary edema" and the release of chemicals which promote inflammation ( cytokines) cause the capillaries ( tiny blood vessels) in the lungs to become leaky causing "non cardiogenic pulmonary edema) both mechanisms can be at work.
