What to do if i have a medium-sized nevus spilus above my knee, and would like to know if it's alright to get a tattoo over it?

Sure: Do continue to keep an eye on both the nevus spilus and all of your skin. A tattoo should be something extremely personal and you're much better off with a nevus spilus -- many people will think it's "cute" or "distinctive for you" -- than a tattoo that you are not 100% sure you want or you may regret. Please wait at least a year or two between the idea & actually getting inked. Best wishes.
