Had gallbladder removed back in march. one of the incisions is still not healed and has a hard grape size lump under it. what could it be?

Depends: Which of the incisions is involved? Need to see your surgeon. Could be a suture reaction, late wound infection is uncommon, very rare to have retained gskkstone under the skin, etc. Needs evaluation with your surgeon.

Call your surgeon: for follow up examination , need examination , could be hernia at port site , or fluid accumulation .

Granuloma / cyst: Could be related to a retained suture or a small collection of fluid. You should see your surgeon to have that evaluated. Can't walk around forever with an open wound.
