Herniated disk?: Could be a ruptured disk in neck, especially since you've had the same symptoms before, which is typical. Did u get ant treatment(Tx) then? Place to start is w/ fam doc for pain meds & referral for MRI. steroids & PT would be initial Tx, unless weakness is present, in which case more urgent. Surgery may be necessary, but most resolve without. If persists or recurs, surgery more likely. .

Suspect: Consider a problem affecting the brachial plexus. Thoracic outlet syndrome could be the culprit. A specialized EMG directly focusing on the plexus could uncover your diagnosis.
