Could mean following: possible sexual excitement without ejaculation which could be relieved by masturbation, so called "lovers nuts" or "blue balls". Might be intermittent testicular torsion, pain would be of sudden onset & testicle would be higher up in scrotum. Test is to twist testis internally & keep turning it, possibly few complete turns, until pain occurs which has 2B identical to your monthly pain.
