Does a brain mri need contrast to look at chirari?

No: Noncontrasted MRI is the test of choice to radiographically diagnose Chiari's. Be aware that there is something called PHASE-CONTRAST MRI or abbreviated PC-MRI. This imaging technique can be used (and is very useful in some cases when looking at headaches associated with Chiari's) to image the malformation. It DOES NOT utilize IV-contrast though the word "contrast" is in the name of the test.
