Usually yes: Contrast causes certain structures and abnormalities to "enhance"(look brighter than on the w/o contrast scan) on an MRI. This can aid greatly in the detection and diagnosis of abnormalities. Without the pre-contrast scan, it may be impossible to determine whether something enhances, because there is nothing to compare it to. So most of the time, a w/o contrast scan is necessary.
