It could: there is no medical definition of "recreational." It is a term used in the popular press for a drug that is taken for anything other than a medical condition where that drug is indicated. So, in essence, it's taking a drug "for fun." Tobacco smoking could certainly be considered recreational drug use, as can drinking alcohol and using opiates for mood-alteration rather than pain treatment.
