What to put under "household income" if separated at doctor's office?

Just your own but?: Your income or household income should not be a factor in your medical care. It is fair to ask you about health insurance, but, about your income. Is suggestive that the dr. May recommend costlier treatment if your income is great. Treatment options and costs should all be presented to the patient irrespective of income. Could be a factor in considering another dr.
