Minutes: nicotine is only present as the parent drug for a few minutes in the bloodstream and brain, and is rapidly metabolized to cotinine, which is mostly inactive. With most drugs of abuse, people need to use much less frequently because they are longer acting, but people take 20-30 puffs of each of 20+ cigarettes in the course of the day - 500 "hits" or more. Also why withdrawal is a few hours later.
