Hard to say: and some think that they really never completely return to normal. For example, it takes months on the original use of opioids for people to develop significant tolerance. That tolerance will dissipate and go back to baseline in most people within 1-2 months. However, if you are re-challenged with opioids in the future, the tolerance that took months will recur in a few days.

See below: It can take up to a year. Naltrexone either as a pill or injection, is an inverse agonist and slows the basilar (unstimulated) mu receptor activity. This seems to hasten normalization to a month or less.
