If a new lesion: Such as an MRI lesion which lights up with gadolinium, this can be actively seen for about 3-4 weeks prior to fading. However, regardless of MRI, there is always pathology even in "normal appearing white matter", unless the inflammatory attack is quelled by potent medications such as Gilenya or Tysabri (natalizumab).
