Many things: Muscles react against gravity and bone density also depends on this. So muscles atrophy, bones get thinner, spine spacing expands (astronauts get taller), there are changes in visual perception and balance. Almost every body system will be altered and if the astronaut is up there for a while, it may take months to reacclimate when they return to earth.
