My left heel hurts really bad with no injury i also have diabetes i don't think it's swelling what could this be ?

Heel pain: Diagnosis depends somewhat on location of pain. If posterior(back) of heel it could be bursitis. If pain more on bottom or near arch then fasciitis is consideration. You are young enough that I wouldn't think diabetic nerve pain would be present. If it persists you should see your physician.
