How could we defferentiate between hypoglycemia caused by factitious insulin injection and by an insulinoma?

Cpeptide test: A fasting C-peptide test could help, but isn't always definitive. A C-peptide test measures native Insulin production and isn't elevated by outside Insulin (injected insulin). So a low or normal level when fasting would be evidence against an insulinoma (insulin tumor). An elevated C-peptide when fasting shows elevated Insulin levels when they shouldn't be, as could happen with an insulinoma
