Had there ever been a disease that has been eliminated altogether from the world?

Smallpox: Smallpox with vaccination and control has been eliminated although there are a few vials of active virus felt to be in the deep freeze in Russia and the US. Polio is close to this and is only stopped by the politics of Africa and Asia.Work is being done also on onchocerciasis with markedly fewer cases currently and the hope soon of elimination.
