Question about vaccines and health forms? are they required by law?

Legal requirement: In new york state, vaccinations for diphtheria, tetanus, pertussis, polio, measles and chickenpox are required before being enrolled in kindergarten. Health forms prepared by your doctor is the only document that will confirm that your child received the necessary vaccines.
