Its normal: You happen to have nipples that are retracted a bit so they seem flush with the surrounding skin. Cold and arousal will stimulate the nipple to expand and then you see the nipple show. Its a normal variant. some people have more prominent nipple than others. Its not bad or a health hazard.

I am a male (38) and for the past couple of years my nipples are always hard. Temperature and arousal aren't factors. No tenderness discharge or anything else atypical. I do take HRT (lowT). I have known other men my age that experience it too?
