Sleep apnea: Daytime fatigue is a very common problem associated with multiple medical problems. The p[roblem becomes "the chicken or the egg"
sleep apnea causes obesity, Insulin resistance, hypogonadism, as well as hypertension.
You should focus on sleep hygeine first and get a 2 step sleep study. The other medical conditions cannot be controlled with untreated sleep apnea.

You're doing fine...: You have made excellent progress towards a more healthy lifestyle. Why do you think something may be going wrong? If you had high blood pressire it is now normal. What is your weight goal? Good idea to take a multivitamin, omega 3 oil and vitamin d=3 and b-12. Be sure you continue to follow up with your physician, continue your weight loss/exercise, and please don't get discouraged!

Psoas muscle sprain?: If all the workup you mentioned was negative, and they were done with the appropriate contrast, you may have a psoas muscle sprain. Consider an osteopathic and/or chiropractic evaluation. Also may be scarring/ recurrence from/of your old hernia repair, which will not show up on imaging.
