May be malignant: Lung masses, especially when associated with spread to lymph nodes, is almost definitely malignant (and serious). He needs an immediate consult with a pulmonologist (lung doctor) and/or oncologist. I biopsy needs to be done, and the treatment based on the results. Don't delay.

Lung mass evaluation: Your grandpa needs to be evaluated for possible lung cancer that has already spread to some lymph nodes and liver. He should see his doctor to get a consult set up with both a pulmonary specialist ( to confirm diagnosis) and an oncologist for treatment if it's cancer. A less likely possibility could be an unusual lung infection ( i.e. Fungus). That is why i'd expect him to first see pulmonary doc.
